Burmese cat: hereditary WNK4 hypokalaemia, screening and care

The Burmese has a reputation as a sturdy breed, yet it carries one well-documented genetic quirk: periodic hypokalaemic polymyopathy, a muscle weakness caused by a fall in blood potassium. A study by Barbara Gandolfi and colleagues, published in 2012 in PLOS ONE, used a genome-wide association approach to pinpoint a nonsense mutation in the WNK4 gene in affected Burmese, making the cat the first known animal model of WNK4-related hypokalaemia (study on Google Scholar). Muscle weakness that comes and goes

Signs usually appear between two and six months of age. The picture described in the 2012 study combines episodic muscle weakness, a characteristic cervical ventroflexion (the cat holds its head down, chin towards the chest, unable to lift it), a stiff gait, muscle tremors, head bobbing and dorsal protrusion of the shoulder blades. The kitten refuses to jump, moves little and tires quickly. Between episodes it may look perfectly normal, which often delays the consultation. Confirming the diagnosis: bloodwork, then a DNA test

Only a vet can make the diagnosis. The workup usually combines a blood potassium measurement, often very low during an episode, with muscle enzyme testing (creatine kinase), typically markedly raised. Other causes of potassium loss, particularly renal ones, must be ruled out first. Genetic testing then settles the question: inheritance is autosomal recessive, two copies of the mutation are needed for the disease to appear, and a simple cheek swab is enough at laboratories offering the test, such as the Veterinary Genetics Laboratory at the University of California, Davis. Long-term treatment and another breed trait

Management rests on oral potassium supplementation, prescribed and dosed by the vet, with regular blood checks: no self-medication and no stopping on your own. Many stabilised cats live a normal life. The Burmese is also heavily represented in feline orofacial pain syndrome: the retrospective study by Clare Rusbridge and colleagues, published in 2010 in the Journal of Feline Medicine and Surgery, covered 113 cats including 100 Burmese, with often one-sided painful episodes triggered by mouth movements and sometimes accompanied by tongue mutilation (study on Google Scholar). Care budget and insurance

The DNA test is inexpensive, in the region of a few tens of euros, and is done only once. The main cost sits elsewhere: consultations and blood panels during episodes, hospitalisation for a severe crisis, then lifelong supplementation and regular laboratory checks. At what age do the first signs appear?

Can two clear-tested parents produce an affected kitten?

Not for this WNK4-linked form, which requires two copies of the mutation. Other causes of hypokalaemia, particularly renal ones, remain possible and justify a veterinary workup.

Can the condition be cured?

No, it is controlled. Potassium can be corrected, the mutation cannot be erased. A well-monitored cat can live a long life, but treatment and monitoring are lifelong.
